York, Inc., not-for profit, is a comprehensive, community-based
mental health organization located in New York City. We provide a
continuum of rehabilitative services to enrich the lives of those
recovering from mental illness and facilitate their transition to
increased levels of independence. Transitional Services for New
York, Inc. envisions broadening its rehabilitative services and
becoming a regional social service provider. Transitional Services
for New York Staff will deliver effective programs with compassion,
integrity, and professionalism. We expect all staff to put our
clients’ needs first while respecting ourselves and each other as
we provide hope to those who participate in our programs. We are
currently looking for a Peer Health Navigator to join our Behavior
Health Care Coordination (BHCC) program located in Jamaica
(Queens), NY. Salary: $40,000 annually, $21.97/Hr. Position Summary
The Peer Health Navigator (PHN) is a full-time position. This staff
is responsible for working with care coordination members in
developing a plan to support recovery and effectively manage future
crises as they occur. The PHN will conduct outreach and enrollment
and facilitate linkages to available community-based support that
address physical health, behavioral health and social determinants
of health. The PHN will work as part of the care coordination team
that provides community support to individuals experiencing
behavioral, medical, and economic crises. Essential Functions Peer
counseling Complete and implement training of peer health
navigation. Assist members in identifying and providing linkages to
community resources, such as vocational programs, outpatient
programs, psychosocial clubhouses, health care services, etc.
Assist care coordinators to track, monitor and assist in members’
attendance of linkages and support services. Escort members to
appointments as needed. Communicate with members’ network to
monitor service needs. Conduct outreach to members and referral
sources to provide education on care coordination services. Visit
members in the community to enroll them in care coordination
services. Document all encounters in progress notes via the
electronic health platforms. Attend staff meetings. Attend special
on-site and off-site training, seminars, conferences, etc. Support
members in the development of a WRAP Plan Complete HRA2010E housing
applications and support members during interviews and tours. Be
able to travel independently by public transportation. Be able to
sit or stand as needed, with or without reasonable accommodation.
May require walking, primarily on a level surface, for short
periods throughout the day, with or without reasonable
accommodation. Be able to reach above shoulder heights, below the
waist or lift as required to file documents or store materials
throughout the workday, with or without reasonable accommodation.
During a declared disaster, assume and adhere to assigned Job
Action role(s) consistent with Disaster Preparedness Plan. Perform
other related duties as required.
Experience/Education/Skills/Abilities Should have at least a high
school diploma or equivalent and have NYS Peer Certification
