Job Description:

The Grand Pavilion for Rehabilitation and Nursing at Rockville Centre seeks a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. This role is pivotal in providing top-tier nursing care and support to our residents, aligning with our commitment to excellence in rehabilitation and long-term patient care. As an LPN at our facility, you will work closely with our team of skilled healthcare professionals to ensure that all patients receive personalized care tailored to their specific recovery needs. Monitor patient health and administer basic patient care under the direction of registered nurses and doctors. Maintain accurate patient records and report any changes in patient conditions to supervising health care provider. Administer medications and treatments as prescribed and monitor for reactions or side effects. Provide emotional support and guidance to patients and their families. Participate in the development and assessment of patient care plans in collaboration with healthcare team members. Proficient in medical software and electronic health records. Strong interpersonal and communication skills. Ability to perform all standard clinical procedures within the scope of their certification. High level of professionalism and patient privacy compliance. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid LPN license in the state of employment. CPR certification preferred.
