Job Description:
Description The Pediatric Hematology/Oncology Clinical
Pharmacist will provide pharmacotherapeutic support to the
pediatric hematology/oncology service of Goryeb Children’s Hospital
and Valerie Fund Children’s Center in both inpatient and outpatient
settings. In addition to serving as a member of the rounding
interdisciplinary care team, the Pediatric Hematology/Oncology
Clinical Pharmacist will take an active role in the education of
patients, caregivers, and members of the pharmacy and
interdisciplinary team(s). This pharmacist will participate in
multidisciplinary committees and will also serve as a preceptor in
the Atlantic Health PGY1 Pharmacy Residency Program. Additional
responsibilities include: Provide comprehensive pharmaceutical care
to pediatric oncology patients, including: review and recommend
optimal, evidence-based medication therapy; provide pharmacokinetic
consultations as required; document any changes in patient and
pharmacy records and inform others of medication changes; prevent,
detect, monitor, document, and report adverse drug reactions and
medications errors; participate in the drug therapy management of
medical emergencies; promote the use of the formulary by converting
non-formulary orders to formulary when possible Participate in
direct patient care rounding and activities in the inpatient
setting Facilitate transitions of care by obtaining patient
medication histories, conducting patient and caregiver education,
and counseling on newly initiated medications Provide education for
pharmacy staff and interdisciplinary team members regarding disease
state management updates, standard operating procedure revisions,
and electronic medical record changes Update electronic medical
record medication orders and Epic Beacon treatment/therapy plan
orders in collaboration with pharmacy and interdisciplinary
colleagues Augment the education of healthcare and non-healthcare
team members on treatments related to pediatric patients; serve as
a preceptor and mentor pharmacy residents and students Engage in
site-based and system-wide multi-disciplinary committees
Participate in the acquisition process for new medications and
liaise with third party payers, as needed, to ensure timely
medication administration Conduct medication use evaluations and
research to identify areas of medication use optimization with the
goal of publishing and/or presenting at local and/or national
meetings Qualifications Education/Experience Must be a graduate of
an accredited school of Pharmacy with PharmD degree required.
Completion of a PGY1 residency and PGY2 residency in Pediatric
Hematology/Oncology or Oncology preferred. Board certification in
pharmacotherapy (BCPS), Pediatrics (BCPPS), and/or Oncology (BCOP)
preferred. Pediatric Advanced Life Support (PALS) Certification
preferred. Current NJ Board of Pharmacy license required. Minimum
of 1-year experience in a hospital practice setting. Experience in
research activities also preferred.
