Job Description:
At Hillcrest Family Services (HFS), we are dedicated to
transforming lives across Iowa by offering specialized human
services. We are seeking a Social Care Navigator to join our team,
helping individuals navigate through our comprehensive programs in
mental health, education, and homeless assistance. This pivotal
role involves assessment, planning, facilitation, and advocacy for
options and services to meet an individual's holistic needs through
communication and available resources. Conduct comprehensive client
assessments to determine needs and goals. Develop effective care
plans and advocate for client access to necessary services.
Maintain regular communication with clients, family members, and
service providers. Coordinate resources and services across
different programs to ensure a seamless support experience. Monitor
and report on the progress of care plans, adjusting as necessary.
Educate clients about their rights and available services. Ensure
compliance with all federal, state, and organizational guidelines.
Strong interpersonal and communication skills Ability to multitask
and prioritize in a dynamic environment Proficiency in case
management software Understanding of the healthcare system and
social services landscape Medical Insurance Dental Insurance Vision
Insurance Life Insurance Certified Professional in Healthcare
Management (optional but preferred) Valid driver's license
